How do I enroll?
To receive services and supports from Adult Services you must:
Examples of disabilities include cerebral palsy, mental retardation, epilepsy, and autism. You will need documents to prove when your disability occurred and have significant limitations in three of seven life skill areas.
Our Community Services Intake Department determines your eligibility. Staff will discuss your skills and abilities with you and your family, and complete the Ohio Eligibility Determination Instrument (OEDI.) The OEDI will look at different areas of your life to see if you need a lot of support or if you can handle things yourself. If you need a lot of support in several life areas, you will become eligible for services from the agency. Services of the agency are provided at no cost to the eligible individuals for as long as it is desired.
