Community Services supports individuals and their families in choosing services and service providers that will best meet the needs of each individual. Services begin after eligibility is determined.

An individual is linked with a Service Facilitator. A Service Facilitator is a staff person who helps an individual with disabilities create a life plan with assistance from the person's family and other people in his or her life. Service Facilitators get to know individuals and families and help them identify dreams for the future. This is an on-going process, not just a one-time meeting.

Individuals who receive services paid for with Medicaid waivers are requires to work with a Service Facilitator. If an individual receives services that are not paid for with a waiver, the individual chooses whether or not to work with a Service Facilitator.

A array of services can be designed to support the individualized needs of each person served. The process begins with a My Plan which is developed by a team chosen by the individual or family, and coordinated by the Service Facilitator.

The Community Services Department manages the enrollment and funding functions that support these choices. It is important that families and individuals understand options that are available to them and seek the support of the Service Facilitator, who can connect them to the funding sources.
