As you get ready for major transition times in your life or the life of a loved one with a disability, the information you find can be overwhelming. Sometimes it might be difficult to know where, or how, to start. The goal of this guide is not to provide an all-inclusive checklist or endless resources. This Transition Guide, created by Hamilton County Developmental Disabilities Services (HCDDS), is meant to help you get started by sharing some of the most important things to consider of life’s major transition times. We hope the information in this guide will help you decide what is important to you and your family, equip you to find the resources that are relevant to you, and help you feel confident as you plan for the future. Each Guide in this series is broken into three sections
The first section identifies a couple of areas where most families will likely want to focus their attention as they begin planning for this life stage transition.
The second section, titled “Other Considerations,” includes additional items a family might want to consider, depending on their personal situation.
The final “More Info” section provides a person you can contact if you have questions as well as a few additional resources that go more in-depth than this guide. These resources are for families who want or need additional information and guidance.
