Preparing to leave high school is a big life change for graduates and their families
For individuals who are eligible for services through HCDDS, our team can guide students and families through the transition years (ages 14-22) and planning for the future.
Our transition consultants work with students, families, teachers and other members of the Individualized Education Program (IEP) team to develop personalized transition plans based on the student’s preferences, interests, needs and strengths. This can include experiential learning to develop life skills that enable a student to achieve post-graduate goals and make connections to community agencies and/or services. Transition consultants also provide information about local training opportunities and resources, as well as steps to guide the student toward future success and ease the transition from school to adulthood.
